Eagle Direct Primary Care Services

Our low flat annual fee covers all these primary care services:

• Annual wellness/physical examinations
• Unlimited sick and follow-up visits
• Electrocardiograms, pulmonary function testing, and audiograms
• Flu Shots
• Limited in-office laboratory testing such as urinalysis, strep testing
• Repair of lacerations
• Minor in-office surgery, cryotherapy
• Psychopharmacology and supportive therapy
• Completions of forms for physicals

What it does not cover:

• Vaccines other than flu – billed to you at my cost
• Attorney letters or completion of forms other than for physicals
• Laboratory and other services performed outside my office
